Marriage stationery
This is the official site of the Australian Marriage Celebrants Program. Anything you read elsewhere that is inconsistent with this site is not accurate.
Marriage celebrants need to obtain the specific marriage forms and certificates (marriage stationery) set out in the Marriage Regulations 1963.
- CanPrint Communications is the Commonwealth’s contracted supplier for marriage stationery and the only authorised supplier of the Form 15 Certificate of Marriage.
- Marriage stationery can only be purchased by registered marriage celebrants. The marriage celebrant registration number must be quoted when purchasing marriage stationery and this will be checked by CanPrint.

- Form 14A brochure ‘Happily Ever Before and After’ can be obtained free of charge from CanPrint Communications.
- The following forms can be downloaded from this website at no cost:
- Form 13 Notice of Intended Marriage
- Form 12C Annual Return
- Record of Use for Form 15
- Template for completing Form 15 Certificate of Marriage.
- Form 15 Certificate of Marriage must be purchased from CanPrint Communications
- Marriage celebrants may generate or purchase marriage forms from other sources but must ensure that such forms substantially comply with the forms specified in the Marriage Regulations 1963.
- If such forms do not comply, the Registries of Births, Deaths and Marriages have the authority under the Marriage Act 1961 to reject them.
- It is not the role of the Australian Government Attorney-General’s Department to approve, authorise or endorse private or commercial products relating to marriage forms and certificates.
- Marriage celebrants need to be aware that forms for marriage are updated or amended periodically.
