Marriage stationery and forms

This page has information on the following:

Marriage equality

On 9 December 2017, the Marriage Amendment (Definition and Religious Freedoms) Act 2017 commenced. The Act changed the definition of marriage to provide for marriage equality in Australia. The right to marry in Australia is no longer determined by sex or gender. Further information is available on the Marriage equality in Australia page.

From 9 December 2017, new marriage forms must be used. If you have received a Notice of Intended Marriage form from a couple who are a man and a woman before 9 December 2017, it remains valid (for 18 months from the date you received that Notice of Intended Marriage form). The marriage forms are available below. More information on the changes to marriage forms for marriage equality, including guidance for the Notice of intended marriage form, is available in the following fact sheet:

Marriage forms

The forms on this page are for official use under the Marriage Act 1961 and Marriage Regulations 2017. Do not modify these forms as this may result in void documents.

All forms can be downloaded from this site, with the exception of the Form 15 Certificate of Marriage. The Form 15 Certificate of Marriage can only be purchased by an authorised celebrant through CanPrint Communications, the authorised supplier of the Form 15 Certificate:

CanPrint is also contracted by the Australian Government to supply other marriage stationery—including marriage registers, official forms and certificates. Your marriage celebrant registration number must be quoted when purchasing marriage stationery.

Visit the CanPrint Communications website for information, order forms and contact details. You can email CanPrint Communications at marriage@infoservices.com.au.

Marriage celebrants may generate or purchase the approved marriage forms from other sources, but must ensure that such forms comply with the forms approved under the Marriage Act. If such forms do not comply, the registries of births, deaths and marriages have the authority under the Marriage Act 1961 to reject them.

Marriage forms consultation

The Attorney-General’s Department is undertaking a review of the official marriage forms under the Marriage Act 1961 and Marriage Regulations 2017. As part of the review, the department invited feedback on proposed changes to the draft Notice of intended marriage (NOIM) form. The period for providing comments closed on 28 October 2018.

The department is currently considering feedback received. An appropriate transitional period will be provided for any changes made to the official marriage forms following the completion of the review in early 2019.

Questions about the official marriage forms, and the review process, can be directed to marriagecelebrantssection@ag.gov.au.
